The prevalence and impact of substance abuse among the rapidly expanding geriatric population are of increasing concern. Substance abuse is especially dangerous for older adults, who experience slower metabolism and elimination. Detection of substance abuse in these clients is complicated by the fact that signs and symptoms of substance abuse may mimic those of other conditions such as dementia and depression. This course provides criteria for diagnosing substance use disorders, particularly those involving prescription drugs and alcohol, and describes useful screening and assessment tools, such as the Short Michigan Alcohol Screening Test–Geriatric Version (SMAST-G) and the CAGE questionnaire. Approaches to engage older adults in treatment are described, and current interventions are reviewed, including individual and group-based approaches. Family involvement in therapy is discussed as well as settings of care for treatment. Nurses will learn about medications to avoid in older adults and medicines that may interact adversely with alcohol. A list of online and community resources is provided. This course is written for nurses who work with older adults and their families in a variety of settings, including primary, secondary, and tertiary levels of care; acute and long-term care; and institutional, home-based, and community settings.
